How to become a patient:
DEO serves only patients who have no medical insurance.
No access to medical insurance? Can’t afford a doctor?
DEO treats patients aged 18 and older who meet the following requirements:

No health insurance, including Medicare, Medicaid, private insurance.

Household income within Georgia DPH guidelines.
-
Residence in Whitfield or Murray County, Ga.

Eligibility Interview annually

At your first appointment allow extra time for paperwork and bring ALL of these documents:

Photo ID
-
2 proofs of residency (rent, utility bill, mail)

Proof of income: recent pay stubs, SSI disability letter, or if living with someone who provides room and board, a note signed by them saying so.

OR a letter from your homeless shelter.
